If I take sure hands and block is that 4 points or 5? (is it only 3 points if you take block twice or any other tier 1 skill?)

It's instances of a specific tiered skill. So the 2nd instance costs more than the 1st. If you're taking 2 different skills the cost increase doesn't apply.
