The concept is fine. Some of the ideas are a bit dull and restrictive though. I'd say this would do:
Roll in pre-game per Ian's suggestion. Automatically roll on the casualty table. Consult the table to see if the implant is successful or not.
Players can choose from the following implants
Eye (2+)- Gain Accurate
Skull (2+)- +1 AV, Bonehead
Legs (3+)- Gain Sprint, Sure Feet
Arms (3+)- Strong Arm, Tackle
Wings (3+)- Gain Very Long Legs, Leap
Exoskeleton (4+)- +1 STR, Thick Skull, Loner
Spine (4+)- +1 AG, Loner
Players immediately increase in value as if they had chosen these skills during the normal skill selection process.
Every implant costs 50k.

Re: Cyborg Implants
I like the idea, I like the 2ed throwback idea but I am concerned that it will be a lot of work to fit that in the pretty well balanced game that LRB6/CRAP has produced.
2ed did not care about balance or playability.

Kick off return was my idea :)
