I think "b" is just the favorite way of anyone because you can choose to heal your star player rather than any other player but I am not sure it must be done like that.

All the injury are done at the same time but the apo cannot see and evaluate all the injuries at once so he goes and sees each injured player one after the other and he knows that after seeing the first one, if he doesn't heal him and go to see another one to see if it is even more serious, he might die (at worst). So you must decide to use it or not (even if you still have to check you star player afterwards) or it will be too late.

I do not say it must be like that but I do think it could be one interpretation. What do you think of that?

It never happens to me.
For your information though, what happens to us once in our league is that we had a dead player due to pitch invasion and we related this rule to a player pushed into the crowd and decided not to have the possibility to use the apo in that case (after writing it on forums we know we were wrong but now we decided to stick with this rule till the end to be fair with the player who had not been able to heal his player).
