What is the difference between Fall Over & Knocked Down?

Page 27 (Fall Over): "When a player Falls Over it is self-inflicted, and it can only happen during a player's own activation:"

Both Fall Over & Knocked Down effectively has the same following rules:
- Turnover (if a player on the active team).
- Become Prone.
- Risk Injury (Armour roll and possibly Injury roll).
- If carrying the ball, it will bounce.

So the only effective difference seems to be if it was caused by the player itself (in the player's activation) or happens at some other time for some other reason (block, wizard's spell, special rule or skill).

However, when looking at Throw Team-mate, this seems to contradict or confuse the two terms.

Page 54 (Landing In An Occupied Square): The player being thrown Falls Over. The player who is hit by the thrown player is Knocked Down.

The thrown player Falls Over outside it's own activation? Arguably not self-inflicted (the thrower caused the result)? Does this even cause a Turnover?

Page 54 (Landing): "If the Agility test is failed, the thrown player will land badly and is considered to have moved involuntarily. The thrown player will Fall Over, as described on page 27. If the thrown player was in possession of the ball, a Turnover is caused. Otherwise, no Turnover is caused."

Does this also apply to the other types of landing (other than a failed Agility landing roll)? The player can land in 5 different ways:
- Successful (empty square).
- Unsuccessful (empty square).
- Occupied Square.
- Crash landing (if the thrown player was Prone/Stunned before being thrown).
- Landing in the crowd (outside the pitch).

Are there any difference between "Fall Over" (as the rules instruct) instead of being "Knocked Down" in any of these cases?
